Full-Time Social Services/Social Worker

POSITION SUMMARY:
Assist with planning, organizing and developing the overall operation of the Social Services Department in accordance with Federal, State, and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Administrator, to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.

EDUCATIONAL REQUIREMENTS & EXPERIENCE:

  •   Bachelor’s Degree in Social Work or a related hospital field required.
  •   Two (2) years’ experience in a hospital, Skilled Nursing Center or other related medical center.

ESSENTIAL DUTIES & RESPONSIBILITIES:

The following duties are normal for this position.  This list is not to be construed as exclusive or all inclusive.  Other duties may be required and assigned.

  • Assist in the development, administering and coordinating of department policies and procedures.
  • Keep abreast of current federal and state regulations, as well as professional standards, and make recommendations, on changes in policies and procedures to the Administrator.
  • Assist with development and implementation of policies and procedures for the identification of medically related social and emotional needs of the resident.
  • Participate in discharge planning, development and implementation of social care plans and resident assessments.
  • Interview resident/families as necessary and in a private setting.
  • Perform administrative requirements, such as completing necessary forms, reports, etc., and submitting such to the Administrator as required.
  • Involve the resident/’family in planning social service programs when possible.
  • Refer resident/family to appropriate social service agencies when the center does not provide the services or needs of the resident.
  • Provide information to resident/families as to Medicare/Medicaid and other financial assistance programs available to the resident.
  • Coordinate social service activities with other departments as necessary.
  • Participate in center surveys (inspections) made by authorized government agencies.
  • Assure that social service progress notes are informative and descriptive of the services provided and of the resident’s response to the service.
  • Meet with administration, medical and nursing staff, as well as other related departments in planning social service programs and activities.
  • Provide written and/or oral reports of the social service programs and activities as required, or as may be directed by such committee(s).
  • Other duties as assigned by Supervisor.

Director of Sales

POSITION SUMMARY:
Under occasional supervision, direct the sales and marketing program at the retirement community; disseminate information to the public regarding the community; perform sales functions: find and follow up on leads, interview potential residents, provide information and sell vacant apartments.
Supervisory Responsibility: Supervise the sales and marketing team of the community that could include the following positions:
Marketing Manager
Marketing Coordinator
Community Relations Manager

Budget Responsibility – Responsible for meeting sales goals, expenses and established sales and marketing budget for the community.

EDUCATIONAL REQUIREMENTS & EXPERIENCE:
Education – High school diploma with some vocational or college course work in business, marketing or a related field; a bachelor’s degree in marketing, social sciences, business or a related field preferred
Experience/Training – Five [5] years of related experience, a bachelor’s degree in marketing, social sciences, business or a related field may be substituted for three of the years of experience; or any equivalent combination of training and experience which provides the required skills, knowledge and abilities.

Full-Time Fitness Center Coordinator

POSITION SUMMARY:
Under general supervision the Fitness Center Coordinator determines the needs of the resident population, and develops and oversees fitness programs for the entire community.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
NOTE: The following are normal functions for this position, and are not to be construed as exclusive or all inclusive. Other duties may be required or assigned.

  • Develops, plans, and implements fitness programs based on resident needs and requests.
    Conducts numerous weekly classes in all levels of living that meet resident needs, and offers alternatives during classes to accommodate different levels of fitness and ability.
  • Conducts numerous weekly classes in all levels of living that meet resident needs, and offers alternatives during classes to accommodate different levels of fitness and ability.
  • Supports Fitness Center and promotes and encourages fitness and healthy living throughout the community.
  • Provides orientation to residents on all exercise equipment, including ongoing monitoring of safe and proper use of equipment.
  • Schedules all Fitness Center classes, and oversees various part time instructor schedules.
  • Supports, promotes and adopts community Masterpiece living and Successful Aging initiatives.
  • Provides oversight of the fitness center environment and equipment, to include: proper cleanliness and maintenance of equipment, safety and storage of equipment, etc.


PERFORMANCE EXPECTATIONS: 

  • Provides effective and courteous service at all times to residents, team members, and guests.
  • Promotes and protects the rights and confidentiality of every resident, and adheres to HIPAA privacy rules and all company policies and procedures.
  • Reports to work on time, is well groomed and properly dressed according to position, and has company name badge on at all times.
  • Ability to teach a wide variety of classes and work a flexible schedule if required


QUALIFICATIONS:

On-Call Concierge

POSITION TITLE: On-Call Concierge

POSITION SUMMARY:
Oversees general concierge and reception tasks. Responsible for interacting in a courteous and friendly manner with guests, residents and staff

  • Assists residents with scheduling transportation services, dining services, and reservations for various community events
  • Answers outside calls as needed
  • Practices confidentiality at all times
  • Acts as first point of contact for all maintenance requests for both residents and department team members. Submits work order requests and inputs information into AOD system in a timely manner

Develops awareness of residents’ interests and needs and assists residents to maximize their independence and participation. Works on a one-on-one basis with residents with dementia as a “Best Friend” providing direct resident care and assist in residents’ activities of daily living and scheduled Memory Support Program activities. Utilizes a positive, can-do attitude. Duties and assignments may be adjusted at the discretion of the Memory Support Director. Provides effective customer service skills meeting the standards and culture expectations of the ABHOW Advantage program.

EDUCATIONAL REQUIREMENTS & EXPERIENCE:
High School diploma or equivalent, vocational/technical training specific to care of elderly with dementia.
Prefer one (1) year experience working with elderly requiring memory support, or any equivalent combination of training
and experience which provides the required skills, knowledge and abilities. Must be proficient in the use of a computer for
input of resident information.
CPR and First Aid certification required.
