MDS Coordinator/Resident Manager/RN

POSITION SUMMARY
Directs the Resident Assessment Instrument process and care planning process to provide and bill for services in the licensed and certified nursing center.  Assists the Director of Nursing to direct health related services for the Nursing Center, as they relate to the MDS / Care Plan functions.

QUALIFICATIONS & EDUCATIONAL REQUIREMENTS:

  • Bachelor’s degree in Nursing.
  • Registered Nursing license in good standing.
  • Experience with the CMS Resident Assessment Instrument process.
  • Knowledge of OBRA/Federal/State regulations.
  • Two years of supervisory experience in a Long Term Care setting preferred.
  • Knowledgeable of standards of care and practices applicable to the delivery of geriatric services, complex medical conditions, multiple medical diagnoses, including preventive measures, rehabilitation, and promoting independence.

ESSENTIAL DUTIES AND RESPONSIBILITIES :

  • Maintains current knowledge base of guidelines for Medicare/Medicaid providers.
  • May conduct pre-admission assessments for Residents seeking admission to the nursing center.
  • Evaluates Residents for meeting the criteria for Medicare payment, to include clinical eligibility and availability of Medicare days.
  • Supervises interdisciplinary team in completing a comprehensive Resident Assessment Instrument, to include Minimum Data Set (MDS), Resident Assessment Protocols (RAPS) and RAP Guidelines.
  • Schedules and certifies on-time completion of RAI process for all Medicare and non-Medicare assessments, according to state and federal guidelines.
  • Monitors resident health status to identify significant changes and complete re-assessment process.
  • Transmits RAI data to Center for Medicare/Medicaid Services (CMS) on a scheduled basis.
  • Communicates with billing personnel to establish Medicare billing accuracy and timeliness.
  • Supervises interdisciplinary team in developing a plan of care for each resident based on assessments.
  • Provides education for interdisciplinary team, licensed nurses and unlicensed staff to document services provided according to the plan of care.
  • Conducts regularly scheduled audits of the services provided by internal and external providers for accuracy and timeliness of documentation to validate billing.
  • Explains procedures and treatments to resident to gain cooperation, understanding, and alleviate apprehension.
  • Assist in planning, organizing, and leading the Health Center in order to insure competent nursing care for the residents around the clock.
  • Be knowledgeable of the established policies and interpreting those policies according to the standards of nursing practice.
  • Works with various clinical services of institution as needed.
  • Participates in community quality assessment and improvements programs.
  • Maintains compliance to all personnel policies, established community policies and procedures, and Federal and State regulations and standards, including but not limited to HIPAA privacy and confidentiality laws.
  • May need to respond as appropriate to personal calls from the Independent Living Center.  Duties in this setting may include providing First Aid, calling emergency personnel (dial 911), and/or calling family members at the request of Independent Living residents.

Full-Time Social Services/Social Worker

POSITION SUMMARY:
Assist with planning, organizing and developing the overall operation of the Social Services Department in accordance with Federal, State, and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Administrator, to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.

EDUCATIONAL REQUIREMENTS & EXPERIENCE:

  •   Bachelor’s Degree in Social Work or a related hospital field required.
  •   Two (2) years’ experience in a hospital, Skilled Nursing Center or other related medical center.

ESSENTIAL DUTIES & RESPONSIBILITIES:

The following duties are normal for this position.  This list is not to be construed as exclusive or all inclusive.  Other duties may be required and assigned.

  • Assist in the development, administering and coordinating of department policies and procedures.
  • Keep abreast of current federal and state regulations, as well as professional standards, and make recommendations, on changes in policies and procedures to the Administrator.
  • Assist with development and implementation of policies and procedures for the identification of medically related social and emotional needs of the resident.
  • Participate in discharge planning, development and implementation of social care plans and resident assessments.
  • Interview resident/families as necessary and in a private setting.
  • Perform administrative requirements, such as completing necessary forms, reports, etc., and submitting such to the Administrator as required.
  • Involve the resident/’family in planning social service programs when possible.
  • Refer resident/family to appropriate social service agencies when the center does not provide the services or needs of the resident.
  • Provide information to resident/families as to Medicare/Medicaid and other financial assistance programs available to the resident.
  • Coordinate social service activities with other departments as necessary.
  • Participate in center surveys (inspections) made by authorized government agencies.
  • Assure that social service progress notes are informative and descriptive of the services provided and of the resident’s response to the service.
  • Meet with administration, medical and nursing staff, as well as other related departments in planning social service programs and activities.
  • Provide written and/or oral reports of the social service programs and activities as required, or as may be directed by such committee(s).
  • Other duties as assigned by Supervisor.

Resident Assistant “Best Friend” Part-Time & Full-Time Positions Open

POSITION SUMMARY:
Under general supervision, actively incorporates “The Best Friends Model” and Las Ventanas’ Philosophy for Memory Support and the CARF/CCAC Dementia Care Standards into the daily rhythm of life of residents with dementia. Develops awareness of residents’ interests and needs and assists residents to maximize their independence and participation. Works on a one-on-one basis with residents with dementia as a “Best Friend” providing direct resident care and assist in residents’ activities of daily living and scheduled Memory Support Program activities. Utilizes a positive, can-do attitude. Duties and assignments may be adjusted at the discretion of the Memory Support Director. Provides effective customer service skills meeting the standards and culture expectations of the ABHOW Advantage program.

EDUCATIONAL REQUIREMENTS & EXPERIENCE:
High School diploma or equivalent, vocational/technical training specific to care of elderly with dementia.
Prefer one (1) year experience working with elderly requiring memory support, or any equivalent combination of training
and experience which provides the required skills, knowledge and abilities. Must be proficient in the use of a computer for
input of resident information.
CPR and First Aid certification required.
